You also have the option of submitting the details of a patient referral to Northwood so we can provide services to a patient. This information may optionally include protected health information (PHI) regulated by HIPAA Security and Privacy laws.

All information provided on the public Referral Form is transmitted securely and is encrypted when you input the information and is transmitted securely and is encrypted when you submit the information to Northwood. The patient information submitted to Northwood will be treated as confidential and will only be accessed and reviewed by the Northwood service team that you designate (either Auto/PIP or Workers’ Compensation).
